Companion Care, Live In & 24 Hour Home Care

   Provides personal care, household assistance and companionship.  This includes day-to-day activities like bathing, dressing, and meal preparation. RN visits offer infection control, patient assessments, and coordinate care as defined by physician orders. Our Team is trained in emergency procedures, home safety, and developing strong communication, behavior management skills, dementia care, fall prevention, and assisting with mobility.

1. Personal Care: This includes day-to-day activities like bathing, dressing, and meal preparation. While individuals may need help with personal care, it does not require a doctor’s prescription and is categorized separately.
2. Household Assistance: Services like light housekeeping, grocery shopping, and laundry fall outside the remit of home health care, as they do not address medical needs.
3. Companionship: Emotional or social support comes with our compassionate caregivers, and Home Team's ability to foster involvement in our community.

Post-Operative Care

Includes patient assessment, pain management, wound care, promoting mobility, and patient education on topics like medication and signs of infection.

Respite Care

 To give primary caregivers a needed rest from the demands of 24/7 caregiving, supporting their well-being. Covers essential skills like basic caregiving, safety, emergency procedures, and communication.
